FOR PERSONS WHO ENTER THE SWEEPSTAKES THROUGH THIS CREATIVE PRESENTATION, THESE RULES GOVERN ALL ASPECTS OF PARTICIPATION AND PRIZE AWARD.
1. Introduction. The Grand Prize Drawing ("Sweepstakes") is sponsored by ePrize, LLC, 34405 West 12 Mile Road, Suite 123, Farmington Hills, MI 48331 ("Sponsor"). Sweepstakes is offered in conjunction with multiple companies ("Promoters") through different Web sites ("Creative Presentations"), announcing many different prizes. One such Promoter is SurveyNetworks.com, 4509 South 143rd Street, Suite 9, Omaha, NE 68137. One (1) winner will be selected in a random drawing from among all eligible entries received through all Creative Presentations. The prize for which an entrant is eligible depends on the Creative Presentation through which the entrant entered the Sweepstakes. Odds of winning depend on the number of entries received in any given Creative Presentation multiplied by the total number of Creative Presentations. The Sweepstakes is offered only in the United States. Void where prohibited.
ONLY ONE (1) PRIZE ACROSS ALL CREATIVE PRESENTATIONS WILL BE AWARDED.
2. Timing. The Sweepstakes begins January 1, 2005 at 12:01:00 a.m. U.S. Eastern Time ("ET") and ends March 31, 2005 at 11:50:00 p.m. ET ("Sweepstakes Period"). Various Creative Presentations may be added throughout the Sweepstakes Period.
3. Eligibility. The Sweepstakes is open only to legal residents of the 50 United States (including the District of Columbia) who are 18 years of age or older at the time of entry. Employees, officers and directors of Sponsor and Promoters, their respective related companies and agents, as well as the immediate family (defined as parents, spouse, children, siblings and grandparents) and household members of each such employee, officer and director are not eligible to enter. By participating, entrant agrees to abide by these Official Rules and the decisions of Sponsor, which are final and binding in all respects.
4. How to Enter. Online: Go to www.SurveyNetworks.com and follow the links and instructions to enter. After you have completed the entire registration form and submitted the required information, you will receive one (1) entry into the Sweepstakes. LIMIT: You will only receive entries into the Sweepstakes after each successful completion of a Survey during the Sweepstakes Period. All entries become the exclusive property of Sponsor, and none will be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by Sponsor. Sponsor is not responsible for lost, late, incomplete, invalid, unintelligible, illegible, misdirected or postage-due entries, which will be disqualified. Use of any automated system or agent to submit entries is prohibited and will result in disqualification. In the event of a dispute as to any entry, the authorized account holder of the email address used to enter will be deemed to be the entrant. The "authorized account holder" is the natural person assigned an email address by an Internet access provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the submitted address. The potential winner may be required to show proof of being the authorized account holder.
5. Selection of Potential Winner. On or about April 21, 2005, Sponsor will randomly select one (1) Promoter's Creative Presentation from among all Creative Presentations. Sponsor will then select one (1) potential winner from among all eligible entries received through the selected Creative Presentation. Entrant selected must comply with all terms and conditions of these Official Rules, and winning is contingent upon fulfilling all requirements. The potential winner will be notified by mail within ten (10) days of the drawing. Except where prohibited, potential winner must sign and return to Sponsor, within ten (10) days of being notified, an affidavit of eligibility and liability/publicity release in order to claim his/her prize. If the potential winner cannot be contacted within ten (10) days of the first attempt to contact him/her or if he/she fails to sign and return the affidavit of eligibility and liability/publicity release within the required time period, he/she will be disqualified, and an alternate entrant will be selected in his/her place at random from among all eligible entries received for the selected Creative Presentation. If a winner has not reached the age of majority in his/her home state, then the prize will be awarded in the name of his/her parent or legal guardian.
6. Prize. One (1) Grand Prize will be awarded. The exact prize for which an entrant is eligible depends on the Creative Presentation through which he/she entered. The Grand Prize for a potential winner who entered through this Creative Presentation will be Fifty Thousand Dollars ($50,000). No substitution, assignment or transfer of prize permitted, except by Sponsor and Promoters, who reserve the right to substitute a prize with another prize of comparable or greater value. Winner is solely responsible for any and all applicable fees and taxes associated with prize receipt and use. Sponsor will withhold and/or report taxes as required by the applicable law of Sponsor and/or Promoter's jurisdiction and winner's jurisdiction.
7. Publicity. Except where prohibited, participation in the Sweepstakes constitutes winner's consent to Sponsor's and SurveyNetworks.com use of winner's name, likeness, voice, opinions, hometown and state for promotional purposes in any media, worldwide, without further payment or consideration.
8. General Conditions. Sponsor reserves the right to cancel, suspend and/or modify the Sweepstakes if fraud, technical failures or any other factor beyond Sponsor's reasonable control impairs the integrity of the Sweepstakes, as determined by Sponsor in its sole discretion. Sponsor reserves the right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Sweepstakes or to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner. Any attempt by any person to deliberately damage Sponsor's or any Promoter's Web site or undermine the legitimate operation of the Sweepstakes may be a violation of criminal and civil laws and, should such an attempt be made, Sponsor and Promoters reserve the right to seek damages from any such person to the fullest extent permitted by law. Sponsor's failure to enforce any term of these Official Rules shall not constitute a waiver of that provision.
9. Release and Limitations of Liability. By participating in the Sweepstakes, entrants agree to release and hold harmless Sponsors, Promoters (including, but not limited to, Silver Carrot, Inc.), their respective officers, directors, employees, parent companies, and agents (the "Released Parties") from and against any claim or cause of action arising out of participation in the Sweepstakes or receipt or use of any prize, including, but not limited to: (a) unauthorized human intervention in the Sweepstakes; (b) technical errors related to computers, servers, providers, or telephone or network lines; (c) printing errors; (d) errors in the administration of the Sweepstakes or the processing of entries; or (e) injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant's participation in the Sweepstakes or receipt of any prize. Entrant further agrees that in any cause of action, Sponsor's liability will be limited to the cost of entering and participating in the Sweepstakes, and in no event shall Sponsor be liable for attorney's fees. Entrant waives the right to claim any damages whatsoever, including, but not limited to, punitive, consequential, direct, or indirect damages. Except where prohibited, entrant agrees that any and all disputes, claims and causes of action arising out of or connected with this Sweepstakes or any prize awarded shall be resolved under the laws of the State of Michigan, U.S.A., without respect to any conflict of law issues, and entrant agrees that such shall be resolved individually, without resort to any form of class action, and exclusively by the U.S. Federal and State Courts of Oakland County, Michigan, U.S.A.
10. Data Collection. Sponsor and Promoters collect personal information from you when you enter the Sweepstakes. The information collected is subject to the Privacy Policy of the site through which you enter the Sweepstakes (http://www.surveynetworks.com/privacy.cfm ) and the ePrize Privacy Policy (http://www.eprize.net/privacy_policy.html).
11. Winner's List/Official Rules. For the name, city and state of the winner and/or a set of Official Rules, send a self addressed, stamped envelope to "SurveyNetworks.com " $50,000 Sweepstakes Winners/Rules Request" c/o ePrize, LLC, P.O. Box 8070, Royal Oak, MI 48060-8070. Requests must be received by July 15, 2005. VT residents may exclude return postage for rules requests.

Thanks for your support. It's good to know that my conversion rates are high. Perhaps the straightforward approach on my http://SQ.4mg.com/prize.htm reassures my site visitors. I look upon your survey as an opportunity for them, not primarily as a moneymaker.
Your example of Tickle.com reveals a quite different purpose, compared to my website. In fact, my page http://SQ.4mg.com/IQtests.htm includes this statement on www.emode.com (Tickle) from Consumer Affairs:

on Tickle.com online IQ test (from http://www.consumeraffairs.com/misc/emode.html ) "I went to the e-mode.com website in answer to an advertisement offering "free" IQ testing. It turns out that taking the test is free, however, to find out the results one had to pay a "one-time fee" of $19.95. In good faith, I provided my credit card information, and received the results of my IQ test. On my billing statement the following month was the fee for the IQ test, which I paid. However, the following month I was billed an additional $19.95." Sarah of Brooklyn (9/18/03)I'm not sure how to increase the numbers of my site visitors clicking through to your surveys. Five of my most popular pages have links leading to it. In its first week Feb10-16, 2005 there were 266 clicks (38/day) to my prize page, leading to 103 click throughs (39%) to your survey, leading to 13 full and 3 partially completed surveys (15.5% conversion). No complaints or comments about having links to your survey have been made to me. I would be happy to continue with that record.
